In the shift to value-based care the coordination between providers, payers, and the patient becomes the cornerstone for delivering better care to make patients healthier. Manifest MedEx, through its partnerships with both providers and payers in the state of California, has brought together data creating a unified patient view and providing value-added services for its participants to advance care improvements.
