This presentation will describe how Northwell Health – a provider network with 18 hospitals and 13,000 providers – is participating in NYS’ Delivery System Reform Incentive Payment (DSRIP) program. Through the program, Northwell is accelerating its transition to value-based payment, and transforming care to better serve the Medicaid population.
The Northwell Health experience focuses on clinical integration, RHIO connectivity, and EHR optimization with our employed providers and community partners. The overarching goal is to improve population health and reduce avoidable hospital use by 25% over 5 years.
